REMARKS

* Printed form authorizing us to collect the payment payable or allocated to us by the contract company on behalf of the client. This form is very important for payment collections

** These are mandatory documents claimed only from companies and individuals engaged in commercial activities.

*** As invoice VAT may be deducted over the VAT payable only from companies and individuals engaged in commercial activities,our service claims VAT in advance despite the insurance firm pays VAT incompletely.

**** Declaration is driver's petition containing the cause, date and place of incident. For companies,corporate seal must be affixed and the declaration has to be signed by a signatory.

*****Official document that can be obtained from any notary public in order that the payment claimed from non-contract insurance clients and made out by insurance firm on behalf of the client may be collected by our service.
